As indicated here, this is the form you need if you are less than 28 years of age. Those who just turn 28 will need to fill the hsmp form for 28 years olds and over.
You will have an obvious age advantage, scoring 5 points straight off because of being under 28 years old. You also have less demand in terms of length of work experience and capacity, and expected salary scale for the last one year is less. So, if age is on your side, go for this form.

If you lost out on 5 points because of your age, do not despair. You may make up for it from your years of work experience or incremental salary progression. This form is the one needed by all who are 28 years and above. Again, please get the Guidance for HSMP applicants 28 years and over
Should you fail to secure the award of a highly skilled migrant program visa, and you really think you have accumulated enough points, and should have gotten the visa, you may wish to re-apply for your HSMP application to be reconsidered.
